Para concorrer a uma vaga ao COMAD, as entidades dever\u00e3o apresentar no prazo de 40 dias, a contar da publica\u00e7\u00e3o do edital (realizada no dia 13 de dezembro), a seguinte documenta\u00e7\u00e3o: estatuto em vigor da entidade com registro junto ao Cart\u00f3rio do Registro Civil; alvar\u00e1 de funcionamento e\/ou CNPJ; documenta\u00e7\u00e3o do representante legal da entidade, que demonstre o seu v\u00ednculo; c\u00f3pia do documento de identidade do representante legal; e of\u00edcio assinado com a indica\u00e7\u00e3o do titular e suplente.<\/p>\n<div align=\"justify\">  <\/div>\n<p align=\"justify\" style=\"margin-bottom: 0cm;\">A entrega da documenta\u00e7\u00e3o dever\u00e1 ser realizada na Coordena\u00e7\u00e3o dos Conselhos Municipais, localizada na Rua Teixeira Soares, 625 \u2013 Centro, das 13h30min \u00e0s 17h30min.
